LGBTQ+ people experience higher rates of substance use and substance use disorders compared to the general population. This is not a coincidence. Minority stress, experiences of discrimination, family rejection, and the painful weight of navigating a world that too often does not affirm who you are all contribute in real ways. Alcohol and substances can become a way to cope with that weight, or to find moments of relief, or to feel at ease in social spaces that would otherwise be difficult.

Recognizing that context does not make recovery easier. But it means that the best support for you is support that understands your whole picture. You deserve care that does not require you to leave part of yourself at the door.

    One of the first and most established LGBTQ+-specific chemical dependency and mental health treatment programs in the US. They offer inpatient and outpatient programs that are affirming of all LGBTQ+ identities. Their 24/7 line is available for referrals, information, and support at any hour.

    Free, confidential, available 24/7 in English and Spanish. This is a treatment referral and information service for substance use and mental health. It connects you with local treatment options and does not provide counseling directly. It is a strong starting point if you are not sure where to begin.

Community-Based and Peer Support

  • SMART Recovery LGBTQ+ Meetings

    SMART Recovery is a science-based, secular alternative to 12-step programs. It uses tools from cognitive behavioral therapy and motivational approaches. LGBTQ+-specific meetings are available both online and in some in-person locations. The approach centers self-empowerment and does not require belief in a higher power.
  • Recovery Dharma

    A peer-led recovery program rooted in Buddhist principles. Recovery Dharma has a strong LGBTQ+ presence in many communities and offers both in-person and online meetings. It emphasizes non-judgment, community care, and personal practice. No fees, no hierarchy.
  • Alcoholics Anonymous and Narcotics Anonymous
    Both AA and NA have LGBTQ+-specific meetings in many cities, which you can find through their national online directories. If the traditional 12-step format is not a fit for you, SMART Recovery and Recovery Dharma offer meaningful alternatives.

What Affirming Recovery Support Looks Like

Affirming recovery support does not ignore the context of your life. It understands that your relationship with substances did not develop in a vacuum, and it does not require you to minimize your identity as a condition of getting help. A good affirming provider or group uses your correct pronouns without prompting, does not treat your queerness or transness as something to address separately, and understands the specific stressors LGBTQ+ people carry.

If you try a resource and it does not feel right, that is information, not failure. Keep looking. A better fit exists.
